Chest Pain: When to Call 911, What to Tell Dispatch and What to Do While You Wait

Quick answer: Call 911 for chest pressure, squeezing, fullness or pain that lasts more than a few minutes or goes away and returns, especially with shortness of breath, cold sweat, nausea, lightheadedness or pain in the arms, back, neck, jaw or stomach. The American Heart Association says minutes matter and that EMS can start care on arrival. Do not wait to see if it passes and do not drive yourself if these signs appear. This is general education, not a diagnosis.

Chest pain is one of the moments when hesitation costs heart muscle. This spoke expands our emergency first aid guide and works with how to call 911 and hands-only CPR and AED basics.

Classic Warning Signs - section banner for Chest Pain

Classic Warning Signs

AHA lists uncomfortable pressure, squeezing, fullness or pain in the center of the chest lasting more than a few minutes or recurring; discomfort in one or both arms, the back, neck, jaw or stomach; shortness of breath with or without chest discomfort; and other signs such as cold sweat, nausea, rapid or irregular heartbeat, unusual tiredness or lightheadedness.

Some heart attacks are sudden. Others start slowly. Pay attention and call 911 even if you are not sure.

Women May Notice Different Patterns

AHA notes women’s most common symptom is still chest pain or discomfort, but women may be more likely to notice shortness of breath, nausea or vomiting, upset stomach, back or jaw pain, unusual tiredness or weakness and anxiety. Do not talk yourself out of care because the pain is not cinematic.

Heartburn Confusion - section banner for Chest Pain

Heartburn Confusion

Heartburn can mimic chest pain. AHA’s practical message: if you are not sure, get checked. Trying antacids while waiting out a possible heart attack loses time.

Why 911 Beats Driving - section banner for Chest Pain

Why 911 Beats Driving

EMS can begin treatment, monitor rhythms and resuscitate if the heart stops. AHA emphasizes that calling 911 is almost always the fastest path to lifesaving care. Passengers who drive a patient cannot perform CPR as effectively on the road.

What to Tell Dispatch From an Apartment - section banner for Chest Pain

What to Tell Dispatch From an Apartment

State that you have chest pain or possible heart-attack symptoms. Give the building address, floor, unit, nearest cross street and any entry codes. Send someone to meet responders if it is safe. Open the door latch so responders can enter. Move pets. Turn on lights. Follow dispatcher instructions and stay on the line.

If you cannot speak, some cities support text-to-911—know whether yours does via local guidance and our 911 explainer.

While You Wait - section banner for Chest Pain

While You Wait

Sit or lie in a comfortable position. Loosen tight clothing. Do not eat. If the person becomes unresponsive and is not breathing normally, start hands-only CPR and use an AED if available, following training and dispatcher coaching. Know where building AEDs live before an emergency.

Medications - section banner for Chest Pain

Medications

Do not start or stop prescription medicines based on internet advice during chest pain. If a clinician has previously told you to take a specific medication for suspected heart attack, follow that personal instruction. Otherwise wait for EMS.

Aftercare Planning for Households

Teach every adult roommate the AHA signs. Post the building address inside a closet door. Keep a medication list ready. People with known heart disease should ask their clinicians for individualized action plans long before symptoms start.
